Botox® remains one of the most recognized and in-demand treatments in the world of medical aesthetics. Known for its ability to smooth frown lines, relax crow’s feet, and prevent the early signs of aging, it has become a go-to solution for both seasoned patients and those new to cosmetic procedures. With growing popularity and widespread social media exposure, it’s no surprise that many professionals across the beauty and skincare industry—including medical estheticians—are wondering if they, too, can provide this high-demand treatment.
If you’re a medical esthetician practicing in Ontario, or if you’re considering entering the field of aesthetic medicine, you’ve likely asked yourself this important question – Can I inject Botox®? And if the answer is no, what opportunities exist for advancing your career in the world of cosmetic injectables?
At APT Injection Training, we specialize in training regulated healthcare professionals through hands-on, clinically guided education in neurotoxin injections, dermal fillers, and regenerative injectable treatments such as PRP, Sculptra®, and Radiesse®. However, we also receive frequent inquiries from experienced estheticians and skincare practitioners looking to expand their scope of practice, transition into clinical roles, or better understand how they can contribute to a successful injectables practice.
This article will explore everything medical estheticians need to know about injecting Botox® in Ontario—from legal limitations to future opportunities, career development options, and how you can still build a thriving role within the growing world of medical aesthetics in the GTA.
Let’s take a closer look at the facts, the rules, and the pathways forward.
Medical Estheticians and Injectables in Ontario – What the Law Allows
Let’s get one thing clear right away – medical estheticians in Ontario are not permitted to inject Botox® or other cosmetic injectables under current regulations. These treatments fall under the controlled acts governed by the Regulated Health Professions Act (RHPA), which means only certain licensed professionals can perform them.
So who can legally inject Botox® in Ontario?
Here’s a list of the regulated healthcare professionals eligible to perform cosmetic injections in a medical aesthetics setting.
- RPN – Registered Practical Nurse
- RN – Registered Nurse
- NP – Nurse Practitioner
- MD – Medical Doctor
- DDS/DMD – Dentist
All of the above must also operate under appropriate medical directives, receive formal injectable training, and adhere to their regulatory college’s scope of practice and ethical standards.
Why Medical Estheticians Cannot Inject Botox® – A Regulatory Breakdown
Even though medical estheticians often play an important and supportive role within the clinic setting—assisting with skincare consultations, performing advanced facial treatments, and educating clients on post-treatment care—they are not classified as regulated healthcare professionals in the province of Ontario. This distinction is critical when it comes to understanding who can legally perform certain controlled medical procedures, including the administration of Botox® and other injectable treatments.
In Ontario, all regulated health professions fall under the Regulated Health Professions Act (RHPA), which clearly outlines what controlled acts are permitted only to those with specific licenses and scopes of practice. Since estheticians are not regulated under the RHPA, they are not legally authorized to perform any medical act that falls into this category—regardless of their level of experience or the setting they work in.
As a result, medical estheticians cannot legally perform the following treatments.
- Administer injections (including Botox®, dermal fillers, PRP, Sculptra®, or Radiesse®)
- Write or act under medical directives
- Prescribe or diagnose patients
- Handle or possess controlled substances
These limitations are not intended to hold estheticians back but are in place to safeguard patient health and ensure accountability. Injectables like Botox® may seem simple on the surface, but they require an in-depth understanding of facial musculature, vascular mapping, pharmacology, dosing, contraindications, and emergency protocols. Even minor errors in injection placement or product selection can lead to serious complications—ranging from facial asymmetry to vascular occlusion.
That’s why Ontario mandates that only regulated healthcare providers, such as RPNs, RNs, NPs, MDs, and dentists, can perform injectable treatments. These professionals are required to maintain licensure with their respective regulatory colleges, stay current with evidence-based practices, and work under medical directives or have prescribing authority, depending on their designation.
Ultimately, this regulatory framework ensures that injectables are delivered safely, ethically, and with the necessary clinical judgment, which is essential in preserving both patient safety and trust in the industry. For estheticians passionate about aesthetics and wanting to grow into clinical roles, there are still excellent paths forward—and we’ll cover those in the sections ahead.
A Career Pivot – How One Esthetician Became an Injector
One of our trainees came to us after spending nearly five years working as a medical esthetician in a high-end Toronto clinic. She was passionate about patient care and loved skincare, but she was also deeply curious about injectables. After researching the requirements, she decided to go back to school and earn her Registered Practical Nurse (RPN) designation.
She returned to APT Injection Training once she was licensed and completed our Beginner Neurotoxin and Filler Training. Today, she combines her strong skincare background with advanced injection skills, providing holistic, science-based treatment plans for her clients. Her story is a perfect example of how career growth in aesthetics is always possible—with the right education and determination.
What Can Medical Estheticians Do in a Clinic Setting?
